Special Needs Patients with Post-Traumatic Stress Disorder Essay Post-Traumatic Stress Disorder (PTSD) is a type of anxiety disorder. PTSD is a serious condition that can develop when a person has experienced or witnessed a traumatic or terrifying event in which serious physical harm occurred or was threatened. Usually the body is able to recover to normal levels of hormones and chemicals the body releases due to the stress. But with a person with PTSD the body keeps releasing the stress hormones and chemicals. An example of PTSD could be a soldier whose been to war. For a person with PTSD, the anxiety and over whelming thoughts of the event can continue and even increase over time. There are three types of PTSD symptoms: Reliving the event, which disturbs day-to-day activity, Avoidance, and Hyper arousal. Treatment for PTSD involves talk therapy (counseling), medicines, or both. The feelings felt by PTSD patients become so strong that many aspects of the individual’s life can be affected. Performing a simple task, like brushing their teeth can become overwhelming (A. D. A. M. Medical Encyclopedia, 2013). Research conducted by periodontists from the University at Buffalo School of Dental Medicine shows that patients with long-term post-traumatic stress disorder (PTSD), showed significant erosion of tooth surfaces. A group of 40 PTSD patients who had been diagnosed with 100 percent disability, were taken from the VA Medical Center and compared to a group of patients that did not have the disorder. The patients with PTSD had significantly more tooth plaque and gingivitis. Doctors suspect that this could be caused because of their illness, the patients are unable to carry out good oral hygiene compared to the non-PTSD patients. Also the PTSD patients were found to suffer from bruxism and clenching which can be related to extra daily stress they feel. Participants received an oral examination and evaluation of tooth wear. Results showed significantly increased wear of tooth surfaces in three dimensions near the gum line, vertical, horizontal and depth in those with PTSD. PTSD patients showed vertical erosion was more than three times greater, horizontal was more than four times greater and depth was more than 10 times greater than the patients not having PTSD (Baker, 2001). Extra considerations need to be taken for dental patients being seen with PTSD. As dental hygienist’s special treatment planning may be needed due to the greater dental and behavioral challenges the PTSD patients may present with. Special attention must be paid to health history questions and getting as much background information of the patients initiating factors, diagnostic criteria, and medical management of the PTSD, so the dental team can better understand and manage the patient. Also, it is extremely important to prevent further loss of tooth structure and surfaces. Together the dentist and hygienist need to work together to rehabilitate the damaged areas. Different recommendations can be made to patient to help with dental issues such as a night guard to help with bruxism and a frequent recall for dental cleanings to help battle the problems caused by poor oral hygiene. At the beginning of the war, Station X had two main goals. The first was to decode the signals sent by the Germans using simple encryption which were easily cracked. The second goal was the mission of cracking the Enigma coding system which the Germans used to send the more vital messages. The first arrivals of Station X were split into two categories; the first were the code breakers, mainly the academic group, which previously helped break codes. The second was the administrative team, which were mostly young girls in their late teens and/or early twenties. The first arrivals were soon joined by the mathematicians. At first, all the departments of Station X were crammed into the main house and some cottages. In September 1939, a wooden extension to the main house was constructed and was called Hut 4, which was later expanded to Hut 8. This housed the Naval section, which were at the time working on decoding non-Enigma messages, however, at the time of the expansion to Hut 8, the Naval section were starting to work on Enigma codes too. By the beginning of 1940, Station X was based around two huts. Hut 6 which would decode the messages then send them to Hut 3 which would translate the messages. When decoding in Hut 6 was finished, the messages were then sent to Hut 3, usually made no sense. Welchman had a system of processes in place within Hut 3, which would decode the Enigma messages when they were able to. Other Huts such as Hut 1 and .. ... Out of all the operations in Europe, none of them need more accurate information than D-Day. In May 1944, Station X discovered that the Germans were expecting a landing in Normandy, but expected that this would be a diversion to the main attack in Calais. Station X managed to find this out and the Allies then manage to adapt their plans. D-Day was originally planned for 4 June 1944 but was delayed due to bad weather conditions until 6 June. Station X was an invaluable source of information about the enemy forces for the last four years of the war. The Allied Commanders still had to achieve the victory, but they used the information that Station X submitted to aid them in their planning. Without the help of Station X, victory would not have been achieved as quickly as it did or may not have been achieved at all.

Everyone in the town is excited because the president was making a stop there and a parade was to be held. The streets were packed as the limousine, carrying President Kennedy, his wife, Jackie, the governor, and his wife, passed by slowly. While the president waved to the crowd, everything seemed to be well and good. Now picture the same scene, a few minutes later: the crowd is in hysteria, no one knows what is going on, but the president has been shot. Nobody knew who did it, or why anyone would, since Kennedy was so well liked among the American public. To this day, the mystery of JFK's death is still unsolved, but the theories are still out there. I believe that Kennedy was shot from behind, and that the government played a part in the assassination. This theory can be supported in many ways. First of all, by looking at photographs and videos of the shooting, it is easily seen that after Kennedy was shot, his head was thrown in a backwards direction. The law of physics which states that every action is matched with an equal and opposite reaction proves that, for this to happen he had to of been shot from the rear. Also, after examining the two wounds in the back of JFK's head and neck, it was proven, through the autopsy, that they were entrance wounds, meaning that he had to have been shot from behind. While this is a common theory among the American public, the thought that the government was involved, is a not such common theory. There are many reasons why I happen to believe that the government played a part in Kennedy's assassination. For one, why would Kennedy be in an open convertible that was going at a very slow speed? Should he not have been in a concealed limousine, shielded by bullet-proof glass? Also, when the autopsy was performed, I would think to have the best, most experienced doctors assigned to examine Kennedy's body, instead, a newer, inexperienced doctor was given the case. Lastly, I do not understand why it has been so long, and the murderer still has not been found. The United States is supposed to be the most advanced country, economically and technologically, in the world, and we can not find out who killed one of our most beloved presidents? This just does not seem right. Overall, JFK was a very well liked man among the American public. Around the time of his murder, no one could understand why somebody would kill him. People, today, still do not understand why, and the mystery is still yet to be solved. Many theories out there, one of which has to be true. With all of this unpleasantness the atmosphere was ripe for some kind of violent outburst in response. It all began in 1692 in the home of Reverend Sam uel Parris. Whose daughter had begun having seizures or spasms, suffering delusions, and had fallen in to a strange trance-like state. It was the doctor that examined her that brought him the side effects of witchcraft as a cause. He suggested that Betty Parris was either a practitioner of the dark craft or a victim of it.1 All of the girls who were companions of Betty, Mary Warren and Abigail Williams, admitted that they had danced and carried on in the woods under the encouragement of Tituba, the Parris’s foreign servant. Soon enough all of the girls, including Tituba are accused of witchcraft; but to save themselves they confess and ask forgiveness. They begin behaving as if other witches and the devil, himself, were attempting to punish them and torment them because they no longer wished to serve the devil. Soon enough this group of girls began naming names of, mostly woman in the community who may be witches. The Courts were established to try those accused. From here it is important to mention that most people associate the events in the story of the Salem Witch Trials through the play the â€Å"The Crucible, â€Å"by Arthur Miller. However, his work changed certain elements for dramatic effect that strayed from the true history a bit. For example, Miller portrays Tituba as more of a slave of African descent. In truth, Tituba was an Indian from the island of Barbados. Another major plot in the â€Å"The Crucible† is the love affair between Abigail Williams and John Proctor, a respected citizen of Salem. It is Proctor’s spurning of Abigail’s attention that caused her to turn on him and accuse his wife of witchcraft. However, in reality, Abigail Williams was only 12 years old; there is no verifiable historical proof that there was any relationship at all between Abigail and John Proctor. The philosophy is that, â€Å"children’s interactions and relationships with other children and adults are a vital component of their learning† (Schiller, 1995). Reggio Emilia schools in the United States, Italy, and other countries around the world differ greatly from standard preschools here in the United States. Some of the key differences and components of the Reggio Emilia program is its role in the community, attention to the environment, and its emergent curriculum which functions as a part of the teachers and the children. Role of the Community The community is a big part of the Reggio Emilia schools. There are regularly scheduled meetings for parents to take part in. These meetings are scheduled in the evening so that working parents are able to attend. The parents and teachers discuss issues regarding school policies, child development concerns, and curriculum planning and evaluations (Wikipedia, 2007). The parents are involved in the whole process of the education of their children. Teachers send home journals of children’s thoughts and ideas expressed in class. The lunch rooms, courtyards and bathrooms are designed in a way to encourage community among all the students (Wikipedia, 2007). Each classroom is connected with a phone, passageway or a window. The classrooms are equipped with art centers called atelier (Gandini, 1993). In the atelier are easels, watercolors, crayons, markers, paper, and any art materials children need to be creative. Emergent Curriculum There is no time schedule where at some part of the day the students are learning about science, then language arts, and then math. For their work period, there is no set curriculum that the teachers must follow. The curriculum is an emergent curriculum which is child centered. Emergent means that is builds upon the interests of the child. The teachers have broad goals but can follow the lead and interests of the children (Schiller, 1995). During the work periods, a teacher will work with one small group on their project while the other students are engaged in self selected activities. The projects are teacher directed and child initiated and begins with the teacher observing the children and asking the children questions on a topic that interests the child. The students then do in depth studies of the content that interests the children of that group (Wikipedia, 2007). The project moves in unanticipated directions. To avoid confusion the teachers help children make decisions about direction of study, how to research the topic, and how to represent the topic (Cyert Center, 2004). The projects can range in length from one week to the whole school year. When the students are content to be complete with their project they depict their knowledge through drawing, sculpting, dramatic play, or writing. At every school, there is a visual art specialist, an atelierista, who works closely with the other teachers and children (Gandini, 1993). The atelierista guides the children into appropriate media choices to complete their ideas and projects. Because art is such an integral part of all activities, the students’ old works, finished and unfinished, are available to them to finish or add on. While they work on their art projects, the teachers talk to the children and write down any thoughts or ideas the students have. They display the students’ discussions with their art work around the classroom and school (Schiller, 1995). In order for this curriculum to work, the teachers must also learn with the students. The teachers take turns with the following roles: observing, note taking, and recording observations between children. After all observations are recorded, they are shared with other teachers and parents in the planning of the students’ projects. It is crucial that teachers become skillful observers of the children. Their observations guide the curriculum planning and implementation (Wikipedia, 2007). Conclusion The Reggio Emilia approach to learning is a constructivist approach where the students learn at their own pace and guide the curriculum. It is all about the children. Treatment Of Clostridium Difficile Infection Health And Social Care Essay As a nurse working in acute medical ward for elderly, I work closely with patients with C. difficile infection. I have noticed the effect of C. difficile infection in elderly can be fatal. C. difficile is a gram positive anaerobic bacillus. They colonise in the oxygen deficient areas of the body. That can cause life threatening conditions, including diarrhoea, colitis and septicaemia and resulting death. C. difficile infection can cause serious illness and a significant cause of patient morbidity and mortality. It is a major cause of hospital acquired diarrhoea. C. difficile infection can cause serious illness and hospital outbreaks .It can cause significant financial burden on NHS. It is estimated that the increased length of hospital stay itself can cause an excess of around  £4,000 per patient. The number of death certificates mentioning C. difficile infection in England and Wales fell by 29% between 2007 and 2008 ,after increasing every year since records began in 1999(National Statistics,2008). According to Weston (2007), Clostridium difficile was first identified in 1935s, but until the late 1970s it was not identified as the cause of pseudo membranous colitis following antibiotic therapy. C. difficile infection is more common in elderly (over the age of 65). People who have a long stay in health care settings, those who have recently had gastrointestinal surgery and those who have a serious underlying illness that compromises their immune system are also at high risk to get C. difficile infection. In-patients are also at high risk if there are hospital outbreaks. Poor infection controls are also an important risk factor. Causes Antibiotics are considered as the most important cause for C. difficile infection. Any antibiotic can cause C. difficile infection, but Broad spectrum cephalosporins, broad spectrum penicillin and clindamycin are most frequently implicated. The second most commonly named antibiotic is Co amoxiclav (Health Protection Agency, 2008). The use of proton pump inhibitors such as lansoprazole, omeprazole and pantoprazole are also potential risk factor for C. difficile infection (Leonard et al., 2007). The disruption of normal harmless bacteria in the gut, because of antibiotic therapy also allows the C. difficile to multiply to greater number. The bacteria start to produce toxins. The antacids suppress the gastric acid secretion and as a result, C. difficile bacteria, including the spores are less likely destroyed. The reason for community associated C. difficile infection was unclear but it is become clear that the reasons for the majority of the infections are associated with antibiotic pr escriptions or hospitalisation (Wilcox et al., 2008). Transmission The transmission is through faecal-oral route. The infected patients acquire the organisms directly from other patients with diarrhoea. The route of transmission may be direct, via the hands of health care workers or via the hands of patients or via the environment. Asymptomatic people who are colonised with C. difficile are also can be able to transmit the disease. About 3% is the colonisation rate in healthy adults, but this increases to nearly 20 % in older people especially in chronic care wards. The spore form of C. difficile can survive in the environment for five months or more on hard surfaces. It is considered that the primary route of transmission of C. difficile infection via healthcare workers hand. Clinical features and pathogenesis The most important clinical feature is sudden onset of offensive smelling diarrhoea during a course of antibiotic or who had antibiotics with in the previous two months. Patients may pass soft or watery stool more than twice daily or in more severe cases more than 20 times accompanied by severe abdominal cramps (Weston, 2007). Abdominal distension, fever and dehydration may also be present in more severe cases. Unless C. difficile is diagnosed, the patients can be miss- diagnosed with irritable bowel syndrome. C. difficile infection is a major health problem worldwide that leads to increased morbidity and mortality. Healthy adults carry around 500 species of bacteria in the colon, 90% of which are harmless (Weston, 2007). C. difficile colitis results from the disruption of normal colonic flora and C. difficile colonises in the oxygen deficient areas of intestine. The spores are able to replicate and produce toxins that can lead to mucosal damage and inflammation. In a healthy adult t he normal colonic flora inhibit the growth and colonisation by C. difficile. The antibiotic therapy may disrupt the normal flora and allow the C. difficile to colonise very rapidly. After colonisation the organisms produces two protein exotoxins( Toxin A, an enterotoxin and Toxin B , a cytotoxin) in to the colonic lumen. These are responsible for diarrhoea and colitis. Toxin A binds to the receptors in the intestine and cause extensive tissue damage, inflammation and oedema. Both toxins posses cytotoxic activity against cultured cells by same mechanisms but they differ in cytotoxic potency, toxin B is generally 1000 times more potent than toxin A and to play a major role in activating inflammatory repose (Weston, 2007). Toxin B is more important than toxin A in the pathogenesis of C. difficile infection in man. According to lab test reports there are 100 different types of c difficile stains. The most recognised epidemic types is ribotype 027.The most important feature of ribotype 027 is hypertoxin production, 10 to 20 times more toxin than other stains. The C. difficile infection caused by ribotype 027 are more likely to be severe with increased complications such as renal impairment, severe colonic dilatation and sepsis (Freeman et al., 2007).The clinical features include increased severity of illness, failure to respond to antibiotics ,abdominal distension. Raised CRP and rising WCC particularly in patients who may have appeared to respond to antibiotics and deterioration in condition and appears to have higher mortality rate. Diagnosis Laboratory studies of stool sample will help to detect c difficile infection. Stool culture will help to detect the presence of difficile with toxin production. Stool enzyme immunoassay (ELISA)will detect both of the toxins ( A or B). For toxin B Stool cytotoxicity assay will be positive.Endoscopy may demonstrate ,but it is the least sensitive for diagnosing C. difficile as compared to stool assays., Sigmoidoscopy alone may not reveal any abnormality if the disease is confined to the right colon. Colonoscopy is more useful. Because of the risk of perforation Sigmoidoscopy and Colonoscopy is contraindicated in patients with colitis (Weston, 2007). Treatment The treatment of C. difficile infection depends on the severity of the illness. At my work place, the patient is closely monitored and isolated. A stool chart is maintained using Bristol Stool Chart. All antibiotics that are not required are stopped. This will help the normal bacteria to thrive again in the gut. If any patient develops C. difficile infection at my work place, we conduct a thorough investigation for the causes and we notify the antibiotic management team to review the patient. The team will review the patient in the ward (rounds Wed/Fri.) or via the phone. There will be a root cause analysis to find why the patient developed C. difficile infection? In some patients fluid and electrolyte replacement and nutrition review may also be necessary. In mild cases of C. difficile infection, patients are monitored for 48 hrs before starting antibiotics. In severe cases, antibiotics may need to be administered immediately. Metronidazole and Vancomycin are the two preliminary ant ibiotics used in the treatment of the infection (Weston, 2007). Usually a 7 to 10 day of therapy is required. Oral metronidazole 400mgs eight hourly for seven to ten days is the first line of treatment. It is contraindicated in women who are pregnant or who are breast feeding. The most recognised side effects of the metronidazole are an unpleasant metallic taste, nausea, vomiting, diarrhoea, abdominal pain, headache, pruritus, rashes, dizziness and reversible neutropenia. Vancomycin is known to cause the spread of vancomycin resistant bacteria. Vancomycin is used for severe, life threatening cases of C. difficile infection. It is also used for patients unable to tolerate metronidazole and failed treatment with metronidazole. Vancomycin is expensive. Oral vancomycin is not completely absorbed or metabolized in the gut and is excreted in the stool unchanged. This is ideal in the treatment of C. difficile infection. The recommended oral vancomycin doses for adults are either 125mg or 500mg four times daily. The use of a rectal vancomycin enema (500mg diluted in 1000ml of 0.9% sodium chloride injection) is also an alternative. A recurrence of symptomatic disease with re infection occurs in 5-20% cases. Management of repeated relapses is more difficult. The options include slow tapering of vancomycin or metronidazole over a period of six weeks and vancomycin combined with rifampicin for seven days. There are also case reports of successful treatments with intravenous immunoglobulin which contains antibodies to c difficile toxins.
